Job Description:
A company is looking for a Telehealth Nurse Practitioner (NP) to
manage a cohort of patients with chronic conditions.
Key Responsibilities:
Provide telehealth clinical assessments and develop treatment plans
for patients
Coordinate care with telehealth nurses and collaborating
physicians
Document patient encounters in AthenaHealth EHR and support new
patient enrollment
Required Qualifications, Training, and Education:
5+ years of Nurse Practitioner (NP) experience, preferably in
telehealth and chronic care management
Completed an accredited FNP program with national certification
Active NP state licenses in 3 or more specified states
Board Certification required
Willingness to obtain additional state licenses as needed
