x000D_The Optometrist cares for patients in the community clinic
setting. The optometrist will be responsible for examining
patients, diagnosing eye and vision problems, testing patients
depth and color perception, and testing patients ability to
coordinate and focus their eyes. The Optometrist will also
prescribe contact lenses and glasses, and diagnose eye problems
within the optometrists scope. This provider will refer to
supportive care as needed and appropriately, and interacts
positively with other members of the CH Staff._x000D
x000D_EXPECTATIONS:_x000D_- Arrives on time and adheres to set
schedule._x000D_- Works flexible or extended hours where
necessary_x000D_- Participates in health center in-services,
listening and respecting others ideas_x000D_- Demonstrates
awareness of, and compliance with, organizational missions and
objectives_x000D_- Use of professionalism and best efforts in your
position._x000D x000D_DUTIES AND RESPONSIBILITIES: _x000D_Health
Care Services:_x000D_1. Examine eyes, using observation,
instruments and pharmaceutical agents, to determine visual acuity
and perception, focus and coordination and to diagnose diseases and
other abnormalities within the scope of optometry._x000D_2. Analyze
test results and develop a treatment plan._x000D_3. Prescribe,
supply, fit and adjust eyeglasses, contact lenses and other vision
aids._x000D_4. Prescribe medications to treat eye diseases if state
laws permit._x000D_5. Remove foreign bodies from the eye._x000D_6.
Chart documentation according to clinic standards using Electronic
Health Record charting, specific forms documentation, referrals and
ordering specific tests. Documentation of acute and chronic
problems, including medication as needed._x000D_7. Follow-up on
specific disease categories and no show evaluations._x000D_8.
Review laboratory, x-ray and referral documents prior to
filing._x000D_9. Health Education coordination for patient
follow-up for specific health education issues._x000D_10. Refer to
approved medical protocols and/or guidelines for treatment._x000D_
_x000D_Consultation and Referrals:_x000D_1. Consultations with
Medical Director for problems beyond standardized procedures.
Consult physicians as needed._x000D_2. Referrals to other providers
as needed including Health Education referrals as indicated per
protocols._x000D x000D_Teamwork Expectations:_x000D_1. Participate
in clinician peer review process._x000D_2. Participate in CQI:
Quality Improvement Meetings and discussions as requested._x000D_3.
Interact with and maintain referral resources._x000D_4. Will refer
to support services (when appropriate) for counseling and
education._x000D_5. Is responsible for providing in-put to Chief
Medical Officer for resource development, staff, facility,
equipment, supplies, etc._x000D_6. Meeting attendance at monthly
General Staff Meeting and Monthly Departmental Clinician
Meeting._x000D_7. Efficient productivity levels according to health
center standards._x000D x000D_MINIMUM
REQUIREMENTS:_x000D_Education: _x000D_- Graduation from an
accredited Optometry Program_x000D x000D_License: _x000D_- Valid
California Optometry License_x000D x000D_Certifications:_x000D_-
Must maintain current CPR/BLS certification_x000D
x000D_Skills:_x000D_- Bilingual (English/ Spanish)
preferred_x000D_- Interest in and willingness to promote individual
acute, chronic and preventable care programs and concepts._x000D_-
Ability to establish a genuine positive interaction with patients
and staff_x000D_- Agrees with and upholds the mission of the health
center with patients and staff
