Job Description:
A company is looking for a Temporary Care Coordinator to provide
case management services to patients and families.
Key Responsibilities
Perform casework duties and provide case management services to
patients, families, and caregivers
Develop and monitor multidisciplinary collaboration of services for
patients
Assist patients and families with personal and environmental
difficulties related to medical conditions
Required Qualifications, Training, and Education
1 year of Long Term Care experience required
Health plan and Medicaid experience preferred
Bachelor's degree in Health & Human Services or a related field
required
All degrees must be from accredited institutions listed on the U.S.
Department of Education College Accreditation database
Degrees obtained from schools outside the U.S. will be reviewed
individually
